Bloated all the time — what it can mean

Being bloated after a large meal is normal. Being bloated every day, regardless of what you eat, is not. The difference is not how much it swells but how predictable it is.

Short answer: chronic bloating usually comes down to one of four things — incomplete breakdown of food, bacterial fermentation in the wrong part of the gut, immune reactivity to certain foods, or altered gut motility. They produce similar symptoms but require different action, which is why general dietary advice so often fails.

Common causes

Incomplete breakdown of food

If the pancreas produces too few enzymes, or stomach acid is insufficient, incompletely digested food travels further down the gut than it should. There it ferments. Typical pattern: bloating regardless of what you eat, strong-smelling gas, loose or greasy stools.

Bacterial fermentation in the wrong place

Bacteria belong in the colon. If they colonise the small intestine they ferment food before it can be absorbed. Typical pattern: swelling within 30–90 minutes of eating, often worse with fibre-rich or sugary food — the opposite of what general dietary advice recommends.

Immune reactivity to foods

Delayed immune reactions to foods can drive low-grade inflammation in the gut lining. Because the reaction appears hours to days after the meal, it is difficult to link to the right food on your own.

Altered gut motility

If the gut's wave-like movements are too slow, contents remain and ferment. Common in constipation-predominant presentations and aggravated by stress, thyroid dysfunction and certain medicines.

Hormonal and cycle-related causes

Many women experience swelling that follows the menstrual cycle. If bloating is cyclical rather than daily, the explanation often lies in hormonal fluid regulation rather than the gut.

When to seek care

The following should be assessed within conventional healthcare before any extended investigation. They are best excluded early and cost nothing to check.

  • Blood in the stool or black stools
  • Unintentional weight loss
  • Anaemia
  • Fever, night sweats or symptoms that wake you
  • Swelling that is persistent and does not vary through the day
  • New symptoms after the age of 50
  • Family history of bowel cancer, coeliac disease or inflammatory bowel disease

What you can do first

Start with primary care if you have not already

Faecal calprotectin and blood counts are free, fast and rule out serious disease. Any further investigation is more meaningful once that is done.

Keep a diary for two weeks — but the right way

Record the time of the meal and the time of the swelling, not just what you ate. The delay is often more informative than the content.

Be careful about eliminating foods on your own

Prolonged self-imposed elimination diets narrow the diet, reduce gut flora diversity and make later investigation harder to interpret.

Note whether it follows your cycle

If swelling is predictably worse on certain days of the month, that is a different investigation from daily bloating.

Frequently asked questions

Why am I bloated all the time even though I eat healthily?

Healthy food is not the same as easily digestible food. Fibre-rich vegetables, legumes and wholegrains ferment strongly if they reach bacteria in the wrong part of the gut or if breakdown is incomplete. The problem is not the food itself but where and how it is broken down.

My tests were normal — why am I still bloated?

Standard primary-care tests are designed to detect or exclude inflammatory bowel disease and bleeding. They do not measure digestion, pancreatic enzyme production, gut flora composition or its metabolic output. Normal tests exclude disease — not dysfunction.

Can the cause of bloating be measured?

Several of the causes can be measured objectively. Enzyme production is assessed via pancreatic elastase-1 in stool, bacterial fermentation in the small intestine via hydrogen and methane breath testing, and gut flora composition via a comprehensive stool profile. Which measurement is warranted depends on the symptom pattern.
