Brain fog — when thinking moves through syrup

Short answer: the brain is metabolically demanding and sensitive to disturbances in energy supply, inflammation and circadian rhythm. The most common explanations for brain fog are insufficient cellular energy production, low-grade inflammation, disrupted circadian rhythm, hormonal change and nutritional cofactor deficiency.

Common causes

The brain is not getting enough energy

The brain uses around a fifth of the body's energy. When mitochondrial energy production has a bottleneck it often shows cognitively first, before it registers as physical fatigue.

What you can do first

Rule out the simple things first

Full blood count, ferritin, B12, folate, TSH and glucose. Iron deficiency and thyroid dysfunction are common, treatable causes of cognitive symptoms.

Map when in the day the fog is worst

Worst in the morning points to circadian rhythm. Worst after meals points to blood sugar. Even across the day points to something more fundamental.

Address sleep before anything else

No analysis compensates for fragmented sleep, and sleep apnoea is both common and treatable.

Note whether it follows your cycle

Cognitive symptoms that arrived alongside changes in periods, sleep or hot flushes often have a hormonal component.

What is brain fog?
